Renew an SPSV Dispatch Operator Licence

Approximately 12 weeks before the expiry of your licence, the Authority sends you a renewal letter, enclosing three forms and some explanatory material.

You must complete and return the forms to the Authority, along with supporting documents and the renewal fee, as described below.

Checking and completing Form DO5 – Application to Renew a Dispatch Operator Licence

Form DO5 – Application to Renew a Dispatch Operator Licence – is already filled in with many of the details already held on file for you. You should check that all these details are correct and up to date; where they are wrong, you should provide the correct details on the form. You must also fill in the details of your current Tax Clearance Certificate, which must be valid on the renewal date.

Tick the boxes in Section 5 and 7 of the form to confirm that all the requirements for your renewal application are satisfied. Then read the terms and conditions set out in section 6, and sign and date the declaration to confirm that you understand and accept them.

Completing Form DO2 – Self-Declaration of Compliance for a Dispatch Operator Licence

Complete Form DO2 as follows:
Section 1 Tick boxes to specify the methods you use to take bookings, and to indicate whether you have systems to track collections and drop-offs, to inform customers of delays, and to retain booking records.
Section 2 Outline the ways in which you try to provide priority services to people with disabilities.
Section 3 Tick boxes to indicate whether you have a system to keep records of all affiliated drivers and vehicles.
Section 4 Tick boxes to indicate whether you have a staff training plan, and, if you have, the areas that it covers.
Section 5 Tick boxes to indicate whether you have a procedure for handling complaints, and, if you have, whether you have a system for retaining records.Outline your complaints procedures and answer the questions in the form relating to specific types of complaint.
Section 6 Read the terms and conditions set out in section 6, and then sign and date the declaration to confirm that you understand and accept them.

Completing Form DO3 – Skills Development Certificate Holder Consent Form

Form DO3 must be completed by the person who holds the SPSV Skills Development Certificate to confirm their consent to their details being used as part of the renewal application.

Please note that the skills certificate holder needs to have passed either the SPSV Entry Test or the Dispatch Operator Test. Certificates for the Industry Knowledge Test will not be accepted.

Submitting your application

When you have the three forms completed, submit them to the Authority at the address below, along with:

  • Your Tax Clearance Certificate
  • The licence renewal fee of €125
